Central Norwich Home Visiting Service is led clinically by Dr Prabhu, the team consists of Paramedic Practitioners and Advanced Nurse Practitioners who visit patients who cannot attend their surgery.
The visits are for acutely unwell patients requiring treatment, and for patients who have long term or chronic health conditions – supporting them in managing these.
he Team seek to enable patients to stay at home to be treated, wherever possible and appropriate, avoiding unnecessary admissions and supporting the patient in recovering without the added stress of a hospital visit. The Team collaborate with the East of England Ambulance Service and Community Teams as well as liaising closely with the Hospital.
